The plugin running on your PC handles that fetch automatically; you don't have to be at the keyboard at the time of the click, but Calibre needs to be open for the request to be picked up.
Yes — the plugin runs inside Calibre, so Calibre needs to be open on your PC when you tap "Listen". The plugin polls every few seconds for fetch requests from your account, uploads the file, and you get a notification when the audiobook is ready. If your PC is off, the request waits in queue until next time you launch Calibre.
